How to submit digital consent form to FIRST

FIRST prefers digital consents for students and coaches. Complete the forms on the www.firstinspires.org website.

1. Coach/Admin will invite team parents/guardians from the team dashboard
2. Parent/guardian is required to create an account (for themselves) and login to complete the National consent form digitally for the child
3. Parent/guardian will need the correct Team Number and Program name to apply to the team. Program is FIRST LEGO League Challenge
4. Access and sign the consent form

The site & consent form can be reached from a smartphone.
Registering for a Parent/Guardian account and signing consent for a child should only take a few minutes.

All Consent Forms for a team need to be complete by the morning of your Qualifier!

If you must use paper consents:

If your team is submitting paper consents, all of the completed consents must be placed in an envelope or folder labeled with the Team Number on the outside. The top document inside must be the official Team Roster printed from the team's National dashboard (even if the team is only submitting paper Illinois consent forms). All consents must be stacked in student name order as printed on the Team Roster. 

Please ensure: 

• No one can casually glance at protected student information. 
  
• Only provide the envelope or folder to the volunteer requesting it at the team's qualifier check in.
